Forms are generally digital questionnaires in which all the answers to a set of questions needed for monitoring your patients are centralized. They apply to each of them individually. They allow you to record specific information, whether it relates to their treatment, their medical particularities, the condition of their dentition, and so on.

| Displays the patient's name. | |
| Allows you to save the form instance. For a new instance, tap the icon again. | |
| Allows you to delete the selected instance. | |
| Enable/disable bar buttons. Also allows you to recover the settings already made from the currently selected desktop. | |
| Closes the toolbar visualization. |
