The forms are generally digital questionnaires, in which the answers are centralized, to a set of questions necessary for the follow-up of your patients. They apply to each of them. They will allow you to record specific information: whether it is information related to their treatment, their medical particularities, the state of their teeth, etc.
They have several characteristics:
- the possibility of creating or modifying them simply, without knowing the programming
- research to find families of patients, using them as a tool with a multitude of criteria
- the possibility of having an instantaneous and global view of a patient through the "Summary of forms" window
To manage the forms, click on the "Forms" button present in the main toolbar.
Kitview forms are independent: this means that you can exchange forms with other practitioners and above all, by downloading or using forms made by other practitioners.

After completing the cells, click on the icon , to save the data. Once saved, the "Form Resume" window show the data selected. If your form is multiple, you can have several tabs with different dates.
